
CCAC e-Referral Physician Process
Select Patient’s Chart from your tracker, select the [Orders] Button from top menu, then [Enter] Button from the main menu that appears.

Look for “Complete Community Care Services Ref” and select it.

If you have specific instructions for nursing staff with regard to the referral to CCAC, you may enter them under “Instructions”, but you may also leave this field blank.

Once ready, click the [Submit] Button, enter your PIN, and press [Enter] on the keyboard or [Save] to save.

A notification will be sent to nursing staff via their status board that a CCAC referral must be completed.
CCAC e-Referral Nurse Process
Under your status board, you will see a notification under Alerts. Note: the Alerts area may be in a different area of the status board depending on your clinical area.

You will also see the physician order under “Orders” if you select “Current”

From “Current” Orders, you may click the circular 🛈 symbol at the end of an alert to then read any additional instructions the physician has provided in the “Order Detail” screen.

To place the referral, under [Orders], Select [Enter] and look up “Refer to Community Care Access Centre”. You may also type in “CCAC”.

You will get a pop-up requesting the ordering physician and method of order. Select the appropriate physician and proceed through the screen.
Nursing/Providers/Allied Health - Fill out all information required in the order.

Once you have completed filling in the required information, you may click [Submit]. This will electronically transfer the order to CCAC’s CHRIS system. Any documentation (medication orders, documents requiring a physician’s signature) which previously needed to be manually sent/faxed to CCAC will still need to be sent manually.
Once the order is complete and all documentation is sent to CCAC if needed, you can then remove the alert from your status board by navigating to the [Document] screen and locating “Complete Community Care Services Ref”. Left-click the NOW cell, and press the [Document] button.

If there are additional instructions from the physician, they will pop up. Read them to confirm you have followed the physician’s instructions. If not, and modifications are required to the order, go back and amend the order before marking this as done.

A screen with a checkmark for the intervention will appear. Press [Save and Exit].

You will then be asked to confirm if you’d like to mark this order as complete. Select “Yes”.
