There are three forms that are needed for a discharge still.

MRP will do medication and discharge orders in system and then print a “Discharge Prescription” (this prescription only includes NEW medications the patient will be leaving the hospital on, not continuing home meds) which they will sign and give to ward clerk or nurse. ** ONLY MRP CAN PRINT THIS, NURSING AND WARD CLERK CAN NOT**
- A “Home Medications List” will need to be printed (in the discharge tab) and this will be faxed with the discharge prescription to the community pharmacy by the ward clerk or nurse.

The nurse will then give a copy of the “home medication list” to the patient and review medications with them.

The nurse will print a “Patient Signature Page” to give to patient (this will include follow up appointments and additional information.
- ****There is a section on this page that the patient and nurse can sign the bottom agreeing that the nurse has reviewed medications with the patient, the nurse signs this and put a copy in the chart.
