Discharge Disposition and Discharge Packet for Nursing

1.From the black navigation bar, click the Discharge button to open the Discharge Plan. 

 

2. The user can review if the Discharge Order has been entered.

3.Ensure that the prescriptions section has been Finalized (done by the provider).

4.When accessing the Discharge Plan, the Patient Disposition will display as “Required” to remind users this is a requirement prior to discharge. 

To update, click on the disposition field to display the list of disposition options available for selection. The dispositions listed are dependent on the type of account. Note: Users are not able to enter the discharge time/date for the account until the disposition has been updated. 

5. Once the disposition has been entered, the Discharge Time/Date field will become accessible. 

The Discharge Time/Date should only be entered once the patient is ready for discharge and is leaving the unit as completing this field will immediately discharge the patient account. 

The registration status will change from ADM to DIS.

 

Printing the discharge packet

To print the Discharge Packet, click the printer icon and select the Print Packet option.

To reprint a section of the packet, such as the home medication report, select the Print by Type option. Note: The Home Medication Report is not printable until the medication reconciliation has been finalized. If the report is not available, check to see that the physician has finalized the home medication list. 

IMPORTANT: The Home Medication Report is meant to accompany discharge prescriptions. The provider must sign the prescriptions.  When faxing discharge prescriptions to the patient’s  community pharmacy, ensure the Home Medication Report is also provided.
