Handoff Routine

The Handoff Routine is for sending/departing nurses to provide a full report to the oncoming/receiving nurse who will then complete the Transfer of Accountability (TOA) intervention on the patient's worklist. This routine will pull patient specific documentation from the patient's chart to meet the requirements of a SBAR (Situation, Background, Assessment, and Recommendations).

Viewing the Hand Off Routine

  1. Click on Status Board to highlight the patient you wish to view and then click on Open Hand Off

 

                                        2.The Hand Off Routine will open displaying (left to right) the Handoff Note and Widgets

 

Creating/Editing a Hand Off Note

                                   

  1. End-Users can then free text into this Hand Off Note 

** Important Note** This note will be saved as part of the Legal Chart

 

 

 

Viewing past Hand Off Notes

  1. Click on the History icon
  2. The Hand Off Note - Selected Visits (current visit) Overlay will display show all past Acute Handoff Notes

 

Continuing from Previous Hand Off Note

  1. Click on New from previous icon
  2. Will open up the Hand Off Note Overlay again and allow end-user to edit or continue from previous note
