Quality and Risk - Submitting an Employee Incident or Hazard

Where a worker experiences an incident (i.e. an event occurs) during the course of their work that resulted or could have resulted in harm (physical, psychological or illness) the worker will need to immediately inform their leader, and document the incident using the Employee Incident form in Expanse. If the worker has identified a hazardous condition or practice that has the potential to cause harm to people or damage to equipment, materials or environment, the worker should immediately notify their leader and complete an Employee Hazard Report form.
