This tab displays the patient’s active problems as well as the past Medical, Surgical, Family and Social History. Any active problems that have been resolved, inactivated, or ruled-out also display.

Note: Patient History falls under the responsibility of Nursing and Providers to review and update. Patient Problems are the responsibility of the Providers to review and update but Nursing must be aware of Active Problems during patients stay.
- For Nursing and Allied Health users, the History & Problems tab should only be used to view information. In order to edit patient history, these users should navigate to the Worklist and locate the PFSH intervention (See tip sheet Nursing Edit/Review of Patient History for further details).
To view the History, select the chevron beside Medical History, Surgical History, Family History, or Social History.

Note: end-users can see when History & Problems were last reviewed or updated. Information pulls across between visits and must be reviewed/updated on admission.
To view the problems, select the chevron beside Active, or Resolved/Inactive/Ruled-out.

Alternatively, the History and Problems can be viewed through the Problems widget which will likely be contained on the reference panel.
