Managing Referrals (Allied Health)

New Referral

  1. A new referral will display on the Referrals Status Board either “By Location” or “All Locations” with a red Flag. The Referral Reason will be beside the flag to indicate the last ordered reason for referral to discipline.

  1. Once the referral has been acknowledged by the allied health professional. Add the patient to “My List”

  1. Go to the Worklist and document the AH Workload specific to your discipline (i.e. PT Workload seen below) that should have been populated from the Referral Order.

 

  1. Update AH Status to “Active” and Save and Exit

 

  1. Complete the AH Workload intervention by clicking on the “Status/Due” column and selecting “Complete” entering Therapeutic Intervention as the Order Source.

 

 

  1. This completed status will change the red flag on the status board to a green checkmark which will fall off after 3 days (reason it is important to add to My List)

 

 

Discontinuing Referral

  1. Once patient is ready to be discharged from caseload, document AH Status as “Discharged” and Save and Exit        

 

  1. On the Status Board, the ‘Followed by’ column will display the PT status as “Discharged”. If patient is discharged from active caseload prior to 3 days the green checkmark will still appear on Referrals list but Status will update to “Discharged” 

 

**Note** the AH status will be viewable on several areas of the chart for multiple disciplines 

(see below for screenshots)
