Leave of Absence (LOA)

GOING ON LEAVE

  1. Physician will place an order for LOA and be viewable as an Ack/Rev order on status board or in Order Management providing a Start Date/Time and End Date/Time

  1. If any medications need to be sent with patient on LOA, a Pharmacy - Leave of Absence (LOA) Prescription will need to be completed/signed and sent to Pharmacy **Follow site policy generally minimum of 24 hours notice needs to be provided to pharmacy before LOA**

On Main Menu find “Pharmacy - Leave of Absence (LOA) Prescription

 

Search for patient using Account # and click OK

Fill out the necessary information and send to Pharmacy

 

  1. Add Leave of Absence to the Worklist and document when the patient is going on leave.

 

  1. Contact admitting so they can put the patient on LOA once departing
  2. Status Board, EMR and Worklist/MAR/TAR headers will update with a bold LOA 

 

  1. Interventions will update Status/Due to Hold

  1. Medications will update to “On Hold” by Pharmacy based on times given to Pharmacist for LOA - if these times change Pharmacy will need to be contacted to change medication Hold times.

 

RETURNING FROM LEAVE

  1. When patient returns from leave, admitting must be contacted again
  2. LOA from Status Board, EMR and Worklist/MAR/TAR will no longer display
  1. When entering the chart will get notification to activate care items 

  1. Click “Yes” to return Worklist items to previous Status/Due and clocks will populate. Clicking “No, Maybe Later” will have this message populate when entering the chart the next time. “No, Don’t Ask Again” will require the nurse to take all worklist items off hold manually.
  2. Document on Leave of Absence intervention from Worklist
  3. If the patient returns at scheduled time, MAR will return medications to the appropriate Status. If the patient returns sooner, notify Pharmacy to resume medications.
