Manage Pt Transfer (Same Account) for Nursing

When a patient is transferred to a different unit(same account), the physician enters “Transfer Orders”.

When a patient is transferred to a different unit(same account), the physician enters “Transfer Orders”. The transfer orders are a grouping of current and new orders that will be processed upon transfer of the patient to another level of care. 

On the Status Board, the nurse is notified of pending transfer orders by the yellow “Tran” flag on the Tran/DC column. Click on the yellow “Tran” flag to be brought to the order activation screen.

The orders routine will default to Current orders. Click the “Transfer/Admit” button (note the yellow flag to indicate transfer orders ready to process) to open the Transfer orders.

Review the Transfer orders and click Process Transfer to activate all the orders.   

All the orders will now be active on the Current tab in Orders. On the Status Board, the “Tran”  flag will be removed.

 

It is best to activate transfer orders as soon as the patient is transferred to reduce errors in ordering. If the Transfer Orders are not processed, any orders that were to be activated “Upon Transfer” will not be processed. For example: medication orders will not flow to the MAR or Pharmacy, transfusion orders will not flow to Blood Bank, diet orders will not flow to Dietary etc.
